Below you will find the forms and documents required for enrollment. Click to download a form. Please call the applicable school(s) to schedule an appointment to enroll your child(ren). School telephone numbers can be found at the bottom of this page.

In addition to the forms above, new students must also provide:

  • Copy of Birth Certificate or Adoption Decree
  • Proof of Wayland Residency (*see below)
  • Copy of Doctor's Physical, dated within the last 13 months, including a record of immunizations birth-to-date

*Proof of Wayland Residency

The following documents are required as a proof of residency. Please note that post office box addresses will not be accepted.

  Home Owners:

  1. Executed Purchase and Sales Agreement with closing date, mortgage statement, or deed, and
  2. Two of the following: utility bills issued within the last 60 days (gas, oil, electric, home telephone, cable), current car insurance statement, Massachusetts car registration or excise tax bill. In the case of families who have not yet moved into the property at the time of registration, these two documents may be supplied within 30 days from the student’s entrance date. Should these documents not be provided, students may be required to withdraw from school. 

    Renters:

  • A formal rental or lease agreement indicating the names of all tenants and the effective dates of the lease. Both the landlord and tenant signatures are required, and
  • Two of the following: utility bills issued within the last 60 days (gas, oil, electric, home telephone, cable), current car insurance statement, Massachusetts car registration or excise tax bill. In the case of families who have not yet moved into the property at the time of registration, these two documents may be supplied within 30 days from the student’s entrance date. Should these documents not be provided, students may be required to withdraw from school.
